WOLVES DEN FITNESS, LLCWaiver, Release of Liability, and Assumption of Risk Agreement
Because physical exercise can be strenuous and carries a risk of injury, Wolves Den Fitness, LLC and its owner, Andrea Ochoa, strongly recommend that you consult with your doctor before beginning any workout program or using gym equipment.
By signing this waiver, you (including clients, guests, and any participating family members) agree that if you take part in any physical activity, whether on-site, off-site, or during a gym-sponsored event, you do so voluntarily and at your own risk.
This includes, but is not limited to:
Use of gym equipment or amenities
Personal training, classes, or fitness programs
Sudden equipment malfunction or dropped weights
Slips, falls, or accidents on the premises (inside or outside)
Contact with other participants
Weather conditions like heat or humidity
Your own physical limitations, fatigue, or inexperience
You are also responsible for any decisions regarding diet changes, supplements, or weight-loss/bodybuilding products. We recommend consulting your doctor before making any dietary changes.
Wolves Den Fitness, LLC is not responsible for any injuries, illnesses, death, or loss of personal property that may occur as a result of participation.
You agree to assume all risks and to release and hold harmless Wolves Den Fitness, LLC, Andrea Ochoa, its employees, contractors, and representatives from any and all liability or legal claims related to injury, property damage, or death. This includes claims arising from negligence, to the extent allowed by law.
You acknowledge that:
You are physically and mentally able to participate in activities.
You are responsible for notifying us of any health concerns that may affect your ability to participate.
You understand this release applies to all activities associated with Wolves Den Fitness, LLC.
If any part of this waiver is found to be invalid by a court, the rest shall remain in full effect.
Cancellation Policy:To avoid future charges, please provide a minimum of 7 days’ notice before canceling your membership.
Acknowledgment:I understand that I am ultimately responsible for my own safety. I have read and understood this agreement, and by signing below, I agree to all terms. I also understand that this release cannot be modified orally.
Cancellation & No-Show Policy
To respect both your time and mine, the following policies are in place:
24-Hour Notice Required: All cancellations must be made at least 24 hours in advance.
Late Cancellation or No-Show: If you cancel with less than 24 hours’ notice or fail to show up for your scheduled session, you will be charged the full session rate. No exceptions.
Rescheduling: You may reschedule your missed session within the same calendar week, subject to availability. If not rescheduled within that week, any additional makeup sessions will be billed separately.
No-Show Policy: If you miss 3 consecutive sessions with/ or without communication, you will be removed from my client schedule.
This policy is in place to protect the integrity of our time together and ensure all clients receive consistent, high-quality service. Please communicate openly and as early as possible to avoid fees and stay on track with your goals.
